How to Book an Appointment?
- Contact the clinic of your choice for your preferred schedule.
- Prepare your consultation fee, valid ID, and passbook.
- Visit the clinic on the day of your schedule.
Documentary Requirements:
- External and Internal Eye Examination.
- Comprehensive Refraction for Adults and Children.
- Myopia (Nearsightedness) Management for Children.
- Prescription Eyeglasses, Contact lenses, and Specialized Spectacle lenses.
- Color Vision Test.
- Industrial Optometry.
- Eye Health Consultation.
- 10% discount on the member’s prescription eyeglasses.
- Complete eyeglass package for only P700.00 only.
- The eye consultation fee differs for each area such as Tacloban City, Abuyog, Palo, and Dulag Leyte.
(Please contact the office on the discounted rates for your reference.) - If a member avails of their prescription eyeglasses, the consultation or examination fee will be free of charge.
