PHCCI-MPC Tacloban
has been implementing a mortuary financial assistance program for over a decade, providing support to the families of regular members in times of death. The Coopagtutulungan Program was designed to offer a low-cost annual premium while fostering camaraderie among members and promoting loyalty to the cooperative. As outlined in its policy statement, the program aims to embody the spirit of cooperativism by encouraging a sense of brotherhood and sisterhood among members.
While the program was created with good intentions, during an inspection conducted by the regulating agency in October 2024, it was observed that the cooperative was engaging in an insurance-type activity. As a result, the agency recommended partnering with a licensed insurance provider to offer approved insurance products and services.
In response, the cooperative has partnered with an insurance company capable of adopting the existing program structure—now providing life insurance with added benefits, while maintaining the same affordable annual premium. This ensures compliance with regulatory requirements while continuing to serve the members’ welfare needs.

Standard Claim Requirments:
- Endorsement letter from the Cooperative
- Original / Certified true copy of Death Certificate
- Claimant Statement (3 copies for claim & must be duly notarized)
- Identification (3 copies for claim & must be duly notarized) if applicable
- Attending Physician’s Statement (If died at the hospital) (3 copies for claim & must be duly
notarized) if applicable - Barangay Certificate (if died at home)
- Marriage Certificate or Birth Certificate of the claimant.
- Affidavit of Two Disinterested Persons (to be submitted if there are discrepancies in the details of the
member/beneficiary in the submitted claim requirements)CENOMAR (of both insured and claimant) and
Notarized Affidavit of Cohabitation (indicating their years of living together), if the claimant is a common-
law-partner

- Accidental Death
- Original copy of Police Report, Incident Report, or Affidavit of Eye Witness
- Accidental Dismemberment Benefit / HIB
- Endorsement letter from the Cooperative
- Original copy of Police Report, Incident Report, or Affidavit of Eye Witness
- Original copy of Medical Certificate indicating final diagnosis
- Original copy of Billing Statement
- Surgical Report (if a surgery was done)
- Picture of the loss of any part of the body of the insured member.
