For a Common-law Partner (amancebado)

1. Certificate of No Marriage (CENOMAR) (original) (both parties)
2. Affidavit of Cohabitation or Article 34 (4 copies)
3. Baptismal Certificate (with ‘For Marriage Purposes’ note.) (original)
4. Confirmation Certificate (with ‘For Marriage Purposes’ note.) (original)
